Camp Forming Ltd. fined $55,000 after worker injured at Concord facility
Camp Forming Ltd., a concrete forming contractor in Concord, was fined $55,000 after a worker was injured while cleaning a moving conveyor at its facility at 71 Creditstone Road. The company pleaded guilty and was convicted on Sept. 12, 2025, in Provincial Offences Court in Newmarket for failing to provide information, instruction and supervision under section 25(2)(a) of the Occupational Health and Safety Act.
The case involves Ontario’s requirement that employers provide information, instruction and supervision to protect workers’ health and safety under the Occupational Health and Safety Act. The court also imposed a 25 per cent victim fine surcharge under the Provincial Offences Act, credited to a provincial fund to assist victims of crime.
Justice of the Peace Rhonda Shousterman imposed the fine following the guilty plea. Crown counsel were Alicia Gordon-Fagan and Emily Jin (Student-at-Law).
The offence occurred on Feb. 28, 2024, at the company’s Concord facility. A worker was cleaning one of the rollers of a Telebelt telescopic boom conveyor while its belt was in motion. The worker’s upper body was inside an opening under the main conveyor system.
When the worker moved to step away while the belt was still running, their coverall caught on the conveyor belt, causing an injury.
A Ministry of Labour, Immigration, Training and Skills Development investigation found the worker had not been trained on the safe use, care and operation of the Telebelt conveyor system beyond the general orientation provided by the vendor at the time the conveyor was delivered to the workplace.
